Process - The Meadows
Prospective residents who have already
visited The Meadows and met with staff need only submit their
Application for Residency to begin the sign-up process. Click the
"Application Form" button below to download and print
a copy of our standard form. Thereafter, please submit your completed
Application for Residency, along with the $300 Application Fee to:

The Meadows at New Horizons

The application fee for The Meadows will be credited in full
to the first-month's fee. Even if you have
not yet visited The Meadows, please feel free to print out
and submit the form now if you wish to begin the application process
before visiting and selecting your unit. The application procedure
typically takes just a few days. Following receipt of a Physician's
Statement, an in-person meeting is required to confirm that The
Meadows will likely be able to meet an applicant's needs and
expectations.
If more than one person is to occupy
the apartment, the additional cost for the second original occupant
at The Meadows is only $225 per month. For the Full-Service
Retirement Program, which includes three daily meals, the second
occupant fee is $625 per month. Leasing staff will be happy to email
floor plans for specific apartments, as they become available from
time to time for new residents.
Any residents of The Meadows
are guaranteed priority access to assisted living accommodations
in the New Horizons community, if the need should ever arise.
